Assistive Technology Templates
These templates can be used by district A.T. teams as a "starter kit" for A.T. Assessments. Each form serves as an outline when conducting assessments, writing the report, and providing training/follow-up.
Use this form after you have received a request to evaluate a student for A.T. Before conducting the actual assessment, it helps to observe the student in his/her school environment.
This template offers a format for writing up your assessment results.
This form can be used to inform teachers/parents about the assessment.
This form can be given to the staff/faculty to fill out to request follow-up services and training based on the recommendations made during the A.T. assessment.
